Home Care Supervisor / Registered Nurse (RN)

The Home Care Supervisor is responsible for the supervision of home care operations and maintaining compliance with all federal, state and PACE regulations governing Home Health Care for PACE Participants. Duties include training and supervision of personal care aide clinical activities, development and implementation of patient care plans.

Licensed Practical Nurse (LPN) – Clinic

The LPN has the responsibility for providing direct and indirect nursing care to assigned participants; manages clinical situations for which he or she is responsible. The LPN will function within the clinic under the supervision of RNs.  He or she will accept assignments dependent upon training and the availability of RN supervision.  Duties include but are not limited to:  completing daily medication passes and reviewing MARs, collecting data for assessments and care planning, evaluating participant responses to nursing intervention, completing clinical task as assigned by RNs, teaching and counseling as assigned by RNs, collaborating with other disciplines and assisting primary care providers with tasks and treatments.  Other duties include communicating effectively with participants, families, staff, pharmacies and vendors, accomplishing tasks in an orderly fashion and working effectively in a team setting.

Bilingual Activities Aide and CNA

The Bilingual Activities Aide under the direction of the Recreational Therapy Coordinator, plans and leads meaningful group and individual activities not associated with treatment plans and monitors the program’s inventory of supplies, crafts, and games for activities. The Bilingual Activities Aide speaks with program participants regarding their interests and hobbies, however, all treatment plans, therapeutic activity plans and treatment interventions are designed and established by a Licensed Recreational Therapist (LRT) to achieve specific participant/client outcomes consistent with the overall treatment plan established by the interdisciplinary team. As a part-time Certified Nursing Assistant, duties include assisting participants with activities of daily living; Assisting with other activities and tasks related to participant care and Center operations under the supervision of a Registered Nurse (RN); Maintains a clean and safe environment.

Registered Dietitian

Responsible for providing nutrition care and education to PACE of Guilford and Rockingham Counties participants.  Evaluate the participant’s nutritional status, develop and implement nutritional care plans, assess nutrient requirements, recommend nutritional prescriptions, evaluate participant tolerance and effectiveness of nutritional care, and communicate nutritional data and information to the physicians and other health professionals.

Home Health Aide

Under the direct supervision of a Registered Nurse (RN), the Home Health Aide (HHA) provides home health care services to PACE Participants. Home Health aides will adhere to PACE Home Health guidelines per training protocols, policies and procedures. Duties in the home include but are not limited to the monitoring and documentation of the Participant’s health condition, providing in-home support and assisting with their activities of daily living (ADL’s).
