PACE of the Triad has doctors, nurses, therapists, dietitians, social workers, pharmacists, personal care aides and activity staff who work as a team to help you stay at home and in the community. You’ll have peace of mind knowing your care needs will be met.

The PACE team supervises, coordinates and delivers care that supports a plan of treatment based on your specific needs and personal wishes.

Convenient Transportation

PACE of the Triad provides transportation to and from our Day Center. PACE also provides transportation and an escort to medical appointments. Our vans are wheelchair-accessible to transport all participants.

Day Health Center & In-home Support

We offer flexible options that can make all the difference in your ability to live independently – including a unique day health center open five days a week. It’s a fun and friendly place where you will find medical services, support services and plenty of activities to keep you busy. You can have meals with new friends, enjoy social activities and get the support you need to make life easier. We also deliver a variety of services directly in your home – from vital medical care to help with routine housekeeping chores.

PACE provides on-call services 7 days a week, 24 hours per day, 365 days a year. We are just a phone call away.
