Please Join PACE of the Triad and other community sponsors for a presentation on “The Partnership for a Healthy North Carolina: A Framework to improve care, service and health outcomes for the most vulnerable North Carolinians“. The presentation will be given on behalf of the N.C. Department of Health and Human Services Secretary Aldona Wos, M.D. and N.C. Medicaid Director Carol Steckel. It will be held Thursday, May 9 from 7:00 – 8:30 p.m. at the Old Guilford County Courthouse, Second Floor located at 301 W. Market St. The event is open to the public, but seating is limited so please come early.

Compliance Coordinator

PACE of the Triad is seeking a part-time Compliance Coordinator to serve as the organization’s Privacy and Compliance Officer. This 20 hour position develops, operates, and monitors the Compliance, Privacy, and Fraud, Waste and Abuse plans and provides oversight of the organization’s Medicare Part D plan and contracts.

Key Responsibilities:

Oversees and manages the day to day operations of the Compliance Program
Drafts policies and procedures for the organization’s Prescription Drug Plan
Routinely reports activities and status of the organization’s Fraud, Waste and Abuse Program
Develops and maintains compliance reporting mechanisms
Develops and implements training programs for Compliance education
Develops provider network, negotiates rates with providers, and renews contracts annually

Qualifications:

Undergraduate degree from an accredited university
Training or experience working with regulatory authorities required
Training or experience working with Medicare Part D programs preferred
Proficiency with Microsoft Office and other computer tools used for data management
Experience facilitating groups

RN Clinical Coordinator

PACE of the Triad is seeking a Clinical Coordinator to manage patient care plan development, implementation, and outcomes monitoring and to supervise the care and services provided by the Interdisciplinary Team and contracted providers.

Key Responsibilities

Supervises Clinical Staff including: home care coordinators, home care assistant, clinic nurses, clinic assistant, social workers, physical therapists, occupational therapists, speech therapists, and registered dietitian.
Provides oversight of the care and services provided by the Interdisciplinary Team and contracted services.
Collaborates with Quality Coordinator to collect and analyze quality data.
Ensures compliance with PACE, state, and local regulations.

Qualifications

Bachelor of Science in Nursing
5 years nursing experience, 2 years management experience, and at least 1 year caring for the frail or elderly
Experience in home care, long term care, or PACE preferred
Current North Carolina Registered Nurse License
Valid NC driver’s license and vehicle

RN Adult Day Health Coordinator

PACE of the Triad is seeking an Adult Day Health Coordinator to provide operational and staff management for the Adult Day Health Center (ADHC).

Key Responsibilities

Provides direct supervision of ADHC staff including: Certified Nursing Assistants, Recreational Therapist, Activities Coordinator, Transportation Coordinator, Volunteers, Receptionist, and Environmental Services.
Manages all ADHC activities pertaining to daily operations including: meals, staffing, transportation, inventory management, and enrollment agreements.
Oversees facility maintenance and repairs and responsible for fire and safety drills and inspections.
Serves as liaison between the ADHC and participants, families, the PACE Interdisciplinary Team, contracted providers, and vendors.
Develops best practices for daily procedures and ensures compliance with all federal, state and local regulations.

Qualifications

Licensed as a Registered Nurse in North Carolina
Bachelor of Science in Nursing Preferred
5 years nursing experience, 2 years management experience, and at least 1 year caring for the frail or elderly
Experience in home care, long term care, or PACE preferred
Valid NC driver’s license and vehicle

Driver

PACE of the Triad is seeking a Driver to transport physically limited passengers to and from their destinations.

Key Responsibilities

Drives a specially equipped van to deliver physically limited persons to varied destinations
Assists passengers on and off the van in a safe and courteous manner
Follows established schedules, adjusts schedules to meet unanticipated changes, determines the safest and most efficient pick-up and delivery routes
Inspects van and maintains it in a clean, orderly and safe condition

Qualifications

High School degree or equivalent
1 year experience transporting individuals with special mobility needs
1 year experience with the frail or elderly
Valid NC driver’s license
Excellent driving record

PACE of the Triad provides transportation to and from our Day Center. PACE also provides transportation and an escort to medical appointments. Our vans are wheelchair-accessible to transport all participants.
