Participant Eligibility:
Individuals who meet the following requirements are eligible to participate in PACE of the Triad:
- 55 years of age or older
- Reside in Guilford or Rockingham County
- Able to live safely in the community
- Meet the State of North Carolina’s Nursing Home Level of Care criteria
How To Make a Referral:
You can actually refer yourself! A referral can also be made by a family member, physician or other provider who is familiar with your condition. To make a referral call (336) 550-4040. For the Hearing Impaired: TTY: (336) 621-2478.
For medical offices and other professionals (social services, home health agencies, etc.), download the attached form, complete and fax to (336) 550-4044.
