Home Care Coordinator Assistant

Job Summary 

Provides clerical support to Home Care Program.  Responsible for working under the general supervision of the Clinical Coordinator. The Home Care Coordinator Assistant also uses discretion to complete responsibilities in a timely, confidential and appropriate manner. Additional responsibilities include communicating home care needs to home care agencies, ordering durable medical equipment, processing invoices and providing customer service to participants/families, vendors and PACE staff. This position also performs receptionist coverage as needed.

Specifications 

Education and Special Training:  High school graduate or equivalent.  2 – 5 years of Administrative assistant experience and/or customer service experience required.

Experience:  At least 1 year of experience working with the frail elderly population.

Licensure, Registry or Certification Required:  Valid NC driver’s license and vehicle.

 Knowledge, Skills, and Abilities: Must be able to work effectively in a team environment.  Act only within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by PACE of Guilford and Rockingham Counties and approved by Centers for Medicare and Medicaid before working independently.  Excellent internal and external customer service skills. Computer skills (Word, Excel, Microsoft Outlook, Data entry). Standard office equipment operation. Must possess excellent verbal communication skills for telephone and in person interaction. Ability to establish priorities and handle workflow with frequent interruptions. Ability to work independently. Flexible and able to work well with others. Ability to control personal emotions. Ability to represent the organization professionally.

Key Responsibilities  

  • Works effectively in a team setting.
  • Communicates effectively with participants, families, staff, and other departments and agencies.
  • Ability to organize, prioritize and accomplish tasks in an orderly fashion. Facilitates achieving customer satisfaction and agency compliance by troubleshooting and helping to resolve concerns.
  • Assists with management of Durable Medical Equipment by ordering special items, processing invoices and maintaining par levels of stock items.
  • Assists with coordination of home care services by
    • Developing relationships with key staff at contracted home care agencies.
    • Sending HCC Plan of Care documents to assigned agencies and communicating changes in the POC to the agency.
    • Processing invoices for home care.
    • Fielding and placing telephone calls regarding scheduling home care aides.
    • Assists with maintenance of HCC records regarding supplies and oversight.
    • Schedules oversight visits for RN to home care agencies.
    • Assists with preparation and maintenance of participant teaching materials.
  • Ensures that all needed supplies and forms are available to staff.
  • Documents observations, treatments, procedures, nursing care and participant activities appropriately.
  • Works with other members of the Interdisciplinary Team (IDT) on coordination of 24-hour care delivery.
  • Maintains medical records according to policies and procedures.
  • Assists with coverage of the front desk as assigned.
  • Abides by all established policies, rules and regulations.
  • Attends staff meetings.
  • Other duties as assigned.
