Medical Social Worker/Case Manager

Job Summary 

Responsible for direct social work case management services to participants of PACE of Guilford and Rockingham Counties program.  Provides screening, psychosocial assessments, care plan implementation and appropriate interventions including, but not limited to counseling, referrals and education.

 Specifications 

Education and Special Training:  Master’s degree from an accredited school of Social Work.

Experience:  Two years of experience in a health related area with a minimum of 1 years’ experience working with the aging, frail and/or elderly; Knowledge of aging specific issues; Knowledge of Medicare and Medicaid required; Computer literate.

Licensure, Registry or Certification Required: NC Licensed Clinical Social Worker (LCSW) preferred or LCSW-A; Valid NC driver’s license and vehicle.

Knowledge, Skills and Abilities:

Must have good interpersonal skills and be able to work effectively in a team environment. Must be able to communicate effectively both orally and in writing; possess the ability to maintain composure during stressful situations; Ability to only act within the scope of his or her authority to practice. Must meet a standardized set of competencies for the specific position description established by PACE of Guilford and Rockingham Counties and approved by CMS before working independently.

  • Have independent mobility in order to climb stairs and walk distances.
  • Utilize correct body mechanics.
  • Read, write and speak in English.

Key Responsibilities  

  • Participates in the Interdisciplinary Team’s (IDT) initial assessments, care planning and periodic reassessments (minimally every 6 months) of participant’s case. Assessments are to be completed prior to the scheduled team meeting.  Communicates participant changes to team members.
  • Conducts home visits to private homes of PACE participants, frequent travel in Guilford and Rockingham Counties.
  • Maintains current written case management records, including periodic reassessment of program participants.
  • Coordinates 24 hour care delivery.
  • Provides individual and family counseling, develops and leads group counseling and activities.
  • Refers participants and families to appropriate community agencies or facilities, acts as liaison with such organizations and as advocates for participants.
  • Encourages effective relationships among staff geared to team building and maintenance of a cohesive team.
  • Consults with and advises staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community.
  • Participates in Program and Policy development of the Social Work component.
  • Documents participant changes appropriately in the medical record.
  • Participates as an active member of the IDT and in participant care planning.
  • Attends staff meetings.
  • Other duties as assigned.
