Intake Outreach Specialist

Job Summary

Under the general supervision of the Director of Community Engagement and Marketing, the Outreach Specialist is responsible for providing community outreach to increase awareness of PACE services and generate referrals. This will include, but is not limited to, formal and informal presentations and meetings, program coordination and the development of other marketing strategies.


Education and Special Training:  Bachelor’s Degree in Communications, Marketing, Human Services or other related program of study.

Experience:  Minimum of 1-year experience working with the aging, frail and/or elderly population. 3 -5 years of experience in a similar role. Knowledge of Medicare and Medicaid required.

 Licensure, Registry or Certification Required:  Must have a valid NC driver’s license and vehicle. Position requires 50% or more travel within specified service areas.

Knowledge, Skills, and Abilities: Must be able to work effectively in a team environment; Must possess excellent communication, interpersonal, pubic speaking and conflict resolution skills; Only act within the scope of his or her authority to practice; Read, write, and speak English; Meet a standardized set of competencies for the specific position description established by PACE of Guilford and Rockingham Counties and approved by the Centers for Medicare and Medicaid Services before working independently. Expert level experience with Microsoft Office and other computer programs (Word, Excel, Access, etc.)

 Essential Job Functions:  

  • Effectively communicates PACE’s philosophy, mission, and services to potential participants, families, caregivers, health and human service agency representatives and other vital community stakeholders to ensure understanding of the PACE model of care.
  • Works with supervisor to create an outreach marketing plan outlining regular contact with hospitals, facilities, senior housing communities, agencies and other entities to ensure effective communication and referral generation.
  • Develops a tracking system that captures appropriate documentation of referrals to promote access, open communication and team awareness. Compiles statistics and maintains records of referrals and outreach opportunities; maintains confidential records and case files.
  • Conducts tours of PACE facility and provides in-person and telephone consultations to potential participants and their families and/or caregivers to include resource information and education materials. Makes referrals to other agencies as appropriate.
  • Schedules presentations, coordinates programs and meetings and participates in health fairs, caregiver support group meetings/events.
  • Establish and maintain professional relationships with identified service providers.
  • Provides weekly, monthly, quarterly and annual reports of statistical information to supervisor and others, as requested.
  • Maintains an up-to-date library of resource information and brochures on commonly requested services.
  • Maintain inventory of promotional items, including order processing and distribution.
  • Prepares a monthly column on topics of interest for seniors to be included in the newsletter and/or for posting on the PACE website or other social media outlets as approved by the Director of Community Engagement and Marketing.
  • Attends and participates in staff meetings, departmental meetings (Intake, Go Team, Interdisciplinary Team) and other related meetings and activities.
