Supporting families, supporting older adults
Family caregivers tell us that their stress goes down, and gratitude goes up, after their loved one has PACE. We get the right specialists, arrange and drive to appointments, and do all the paperwork. PACE takes care of health issues quickly. This lowers the risk of small problems becoming bigger.
We help you care for your loved one in their home. Professional care aides visit participants who need extra support to stay safely at home. Nurses make house calls when your loved one is unable to visit the PACE center.
PACE is healthcare + insurance
PACE provides your loved one’s primary medical care. Our doctors and nurse practitioners spend time with your loved one to really get to know their history, needs and goals. We are great listeners.
PACE includes all your loved one’s medical care — primary care, specialists, medical devices, equipment, supplies, medications, rehab and hospitalizations. You can put your mind at ease knowing that all your loved one’s medical needs are included in PACE, because PACE becomes your insurance plan.
Care plan
Your loved one will have a complete health review when they first enroll. Then, their review will be every 6 months, or more often as their needs change. After these health reviews, we create or update the care plan, which supports independence, dignity and health.
Care plans encourage your loved one to be as independent as possible and keep their minds engaged.
Caregiver involvement
PACE includes you in care plans. The care plan is a 3-way agreement with you, your loved one, and us. Your input and agreement with the care plan makes our partnership successful. You are kept up-to-date about appointments and new health issues.
You can call, email or visit us to ask questions about medical, behavior or social needs. You are invited to join our educational programs that reduce your stress and give you skills to make care smoother. Your loved one’s care plan may include home care. Your continued involvement remains important.
What’s not included
There are some things not included in PACE. For instance, home remodeling is not provided. Transportation for personal outings, or taking your loved one to the grocery store is not a service we can provide. Payments for housing, rent or utility bills are not included.
PACE social workers will help you connect with other available community resources for services beyond what PACE provides.
