Philosophy of PACE
Our goal is to keep older adults living in their community, safely, for as long as possible.
- The Program of All-inclusive Care for the Elderly (PACE) is unique in its team approach to care.
- Our center houses a medical clinic and an adult day program.
- We drive our participants to and from their residence to our center, and to all PACE-approved medical appointments.
Care Team for Older Adults
When you refer to PACE, your patient will have a primary care provider who specializes in older adults.
- Our doctors and nurse practitioners are part of an inter-disciplinary team that also includes physical, occupational and speech therapists, a dietician, social workers, home care aides, transportation and activities directors.
- The Inter-disciplinary team reviews care plans every 6 months, or more often as needs arise.
- This team is supplemented by a full network of specialists and area hospitals.
Quality of Care
When you refer to PACE, your patient will receive coordinated, integrated care from our inter-disciplinary team (IDT).
- “The Gold Standard of Senior Healthcare” – that’s what the American Society for Aging calls PACE
- We provide fast-response preventative care and same-day or next-day primary care appointments as needed.
- Lower hospitalizations and readmissions – nationally, PACE has a 24% lower hospitalization rate, and a 16% lower re-admission rate, compared to Medicaid/Medicare enrollees in nursing homes.
- Reduces depression quickly – nationally, 27% of new PACE participants are clinically depressed, but after 9 months of PACE enrollment, only 5% are clinically depressed.
Family Involvement
PACE is a 3-way partnership between the participant, their family or their caregiver, and us.
- PACE involves the participant and their family in the care planning process and ongoing communications.
- An Advisory Council of participants and family caregivers meets with our Board President and CEO to share direct feedback and ideas.
- 9 out of 10 family caregivers would recommend PACE of the Triad to another family caregiver.

PACE of the Triad consistently ranks above the national average of all PACE organizations. We receive high satisfaction scores from an independent survey:
- 98% of participants and family caregivers said the participant feels engaged and socially connected.
