Program of All-inclusive Care for the Elderly
PACE is called the Gold Standard in Senior Healthcare by the American Society of Aging. PACE provides for all the medical needs an older adult could have. Instead of piecing together services offered by different providers — for example, senior centers, healthcare plans, home care and transportation companies. PACE provides it all. Our team is so coordinated, they meet several times a week to review our participant’s care plans, and make any needed adjustments and changes. It is this coordination of many different professionals — called the Inter-Disciplinary Team — that is the key to PACE’s success.
Leading the way to better lives
PACE stands for Program of All-inclusive Care for the Elderly, established in the 1970s and today serving 60,000+ people in 32 states. Among our peers, PACE of the Triad is a recognized leader:
- Across the country, PACE excels in participant satisfaction. Among this admirable group, PACE of the Triad is above-average in satisfaction with care, communication, responsiveness and engagement.
- We were chosen to share our “best practices” with other PACE organizations nationwide.
Healthier and happier
PACE responds quickly to a participant’s changing social, health and emotional needs. This fast-response and “all-in-one” team work helps us keep older adults happier and healthier than a typical healthcare plan. The numbers tell the story:
- 8% Healthier: PACE has a 24% lower hospitalization rate, and a 16% lower re-admission rate, compared to Medicaid/Medicare enrollees in nursing homes.
- 22% Happier: 27% of new PACE participants are clinically depressed at enrollment. After 9 months of PACE support, only 5% were clinically depressed.
A better future for older adults in North Carolina
Winston-Salem is the future location for a PACE Center that will serve residents of Forsyth, Stokes, and Surry Counties in North Carolina.
